For New Patients:  PRIOR TO SCHEDULING, please contact me at [email protected] so I can send you your office intake forms.  Once these forms are completed and returned, you may then schedule your New Patient visit and might consider scheduling a 30 minute follow up visit two weeks later, to be sure you will be able to be seen.
